After completing this form, print out and submit your application entry along with your video explaining why you should be selected as TODAY's wedding couple. Your completed form and video must be received by 5:00 pm June 30, 2006. However, the sooner you submit your application and video, the better, as we'll stop reviewing entries after we find our preliminary group of couples. We look forward to hearing from you.
