'Teen Mom' Amber: 'I am going to rehab'

Troubled "Teen Mom" star Amber Portwood wants to get her life back on track after a suicide attempt on June 14.

"I am going to rehab for anger control issues and depression," Portwood told Star magazine. "I'm on medication at the moment to just help me relax. I'm in a vulnerable state right now."

A source told Life & Style last week that the 21-year-old star of MTV's hit docuseries tried to take her own life after an argument on the phone with her ex-fiance, Gary Shirley. "She said she was mad because he cheated on her," the source revealed.

In a call to 911 after their fight, Shirley, who is the father of Portwood's daughter Leah, can be heard telling the dispatcher that his ex had threatened to kill herself. "She had said something to me, that she was having a hard time with her life, that call the police, 'They're going to find my body in the garage,' " Shirley told the operator.

Emergency workers found her going in and out of consciousness at her home in Indiana, according to Star. A source also told the magazine that Portwood admitted to hospital workers that she had taken pills. The hospital put her on a 72-hour hold for a psychiatric evaluation. She was released Friday.

Portwood told Star that she'll be headed to sunny California for treatment, but "before I go ... I'm pursuing help with a psychiatrist."

The new issue goes on sale this week. The new season of "Teen Mom" premieres July 5.
